Hours & Scheduling
• All sessions are scheduled by appointment.
• Evaluations are 50+ minutes in length.
• Treatments last from 30 to 90 minutes, depending on what is needed.
Payment
& Billing
Acceptable methods of payment are cash, checks, and credit cards.
Lizanne provides you with a superbill/paid invoice and you are responsible for submitting your insurance forms to your carrier. Most PPO’s cover a portion of the treatment, however full payment is expected on the day of services. Any subsequent insurance payment will come directly to you.
Lizanne Pastore is not an HMO provider.
Cancellation
Policy
We all have busy schedules, and Lizanne understands that
emergencies can arise. However, to be fair to all clients,
there is a minimum 48-hour notice to cancel or reschedule.
The full fee is charged for missed appointments or those not
cancelled within the 48-hour period.
Privacy
P.O.S.T. ~ Wellness by Design and Lizanne Pastore PT, MA, COMT comply with all Federal and State guidelines for patient privacy. All of your personal and medical information is kept as sacred and all medical records are confidential. A copy of the privacy policy is available to all clients. |
