Parking is on the street, allow an extra few minutes.
The closest MUNI bus line is the Noriega 71.
The MUNI street car is the N-Judah, which stops on Judah Street,
a few blocks away.

Hours & Scheduling
The office is open Tuesdays through Fridays by appointment
only. Please phone or email for an appointment. Early morning
and evening appointments are available.
Payment
& Billing
Acceptable methods of payment are cash, checks, and money
orders. Credit cards are not accepted at this time.
Lizanne
will gladly submit your insurance forms on your behalf. Most
PPO’s cover a portion of the treatment, however full
payment is expected on the day of services. Any subsequent
insurance payment will come directly to you.
Lizanne
Pastore is not an HMO provider.
Cancellation
Policy
We all have busy schedules, and Lizanne understands that
emergencies can arise. However, to be fair to all clients,
there is a minimum 48-hour notice to cancel or reschedule.
The full fee is charged for missed appointments or those not
cancelled within the 48-hour period.
Privacy
Lizanne Pastore PT, MA, COMT complies with all Federal and
State guidelines for patient privacy. All of your personal
and medical information is kept as sacred and all medical
records are confidential. A copy of her privacy policy is
available to all clients. |
